Transport assistance

If you are frail, aged or have a disability and live in the City of Swan then Community Care Services may be able to assist you to access community transport for the basics of shopping or attend medical and other appointments at an affordable cost. A little help from community care transport services can support you to retain your independence and keep you connected to your community.

Bookings are taken for Monday to Friday trips with city appointments possible between 9.30am – 3pm. Weekly shopping buses offer a door to door service to your local shopping centre with the bonus of meeting new people. Our friendly volunteer drivers provide a friendly smile and a relaxing trip in one of our accessible and comfortable vehicles. This service is suitable for people using wheelchairs or walkers and if required your carer can accompany you free of charge.

For information about eligibility criteria and the assessment process, contact My Aged Care on 1800 200 422 or visit the myaged care website, or contact NDIS on 1800 800 110 or visit their website.

Mates Place for You

Fellas, are you over 65 and looking for a Mates Place to relax with a cuppa and share a yarn?  Perhaps you just want to get out of the house and mix with some mates? Located right next door to the Altone Community Mens Shed, our Mates Place is where you can meet up and socialise each Wednesday.

If you enjoy gardening we have a lovely space that you are welcome to potter around in or even become involved in some cultivation of vegies or herbs. The alfresco area overlooking the park is a great space to enjoy the outdoors or cook up a barbie lunch. Members can take it easy or participate in a variety of activities at a level to suit them. Perhaps a chess or dominoes challenge takes your fancy, challenge a mate to a darts or team up for a table tennis or indoor bowls. The day’s activities will be guided by you, the member.

City of Swan also offer other clubs for men on Wednesdays including the weekly Altone Community Mens Shed or monthly Blokes Bus outing group to interesting venues and lunch.

Our Mens Groups are open to men who are eligible for community care services. Fees cover door to door transport and morning tea with members paying for lunch. Our fully accessible community bus will transport you to and from home arriving for morning tea about 10am and departing about 2.30pm.

For more information, call the Community Care Services on 9267 9055 Monday to Friday, 8.30am to 4.30pm.
