1.0 SUPERVISION. Children under the age of 10 must be accompanied in the centre by an adult at all times. This includes the entire class duration.
2.0 MINIMUM NUMBERS. All classes must have a minimum number enrolled in order for them to operate. If you enrol for a class that does not achieve the minimum enrolment, you may be offered an alternative option.
3.0 TEACHER/CLASS PREFERENCES. We cannot guarantee the same instructor will teach your child every term, or that you will be offered your preferred lesson day and time when enrolling.
4.0 LESSON ENTRY. Each child must marked as attending by their instructor at the start of the class.
5.0 PAYMENT. Fees are to be paid prior to the start of the first class, in order for the enrolment to be processed. Payments can be made in person or over the telephone using a credit card. No part payments for enrolments can be taken. City of Swan does not offer discounts for concession card holders or families enrolling multiple children.
6.0 RE-ENROLMENTS. Students currently enrolled are given priority enrolment for the following term. Please note that it is your responsibility to re-enrol, Ellenbrook Sports Hub does not automatically carry bookings from term to term.
7.0 MAKE-UP LESSONS. Make up lessons are only available for absences due to illness and are subject to class availability. A maximum of two (2) make up lessons in total are permitted per term. Make up lessons must be used within the term period and cannot be credited, refunded or carried over to following terms. If an appropriate make up class is not available the make-up lesson is void. One (1) make-up lesson is permitted without a medical certificate. In this case you must provide notification prior to the start of the lesson. To obtain two (2) make up lessons a medical certificate must be provided for one of the missed classes by one of the following: Medical Doctor, Physiotherapist or Dentist.
8.0 REFUNDS AND CREDITS. Requests for refunds and credits must be made in writing and will only apply where you or your children withdraw from the program for the remainder of the term. Refunds and credits will only be provided under extenuating circumstances that will disallow the participant from continuing lessons. Documented evidence is required for all refunds (e.g. medical certificate). All refunds will incur a 20% cancellation fee. Credits will be valid for 6 months only. In the event of inclement weather all efforts will be made to ensure the class runs at it’s allocated time through adjustment of location and structure. In the event of a class being cancelled, alternative arrangements will be advised at the time which may include (but are not limited to) different times or pro-rata refunds for the value of one class.
9.0 PHOTOGRAPHY. You are permitted to take photos of your own child only. There cannot be any other patrons in the background of your photo. Camera usage is not permitted in the Centre’s change rooms or toilet facilities under any circumstance. By participating in the class you agree that your image can be used for promotional purposes unless staff are advised otherwise.
10.0 ALL OTHER CENTRE ‘CONDITIONS OF ENTRY’ ALSO APPLY. You are also required to comply with the Leisure Centre Conditions of Entry(PDF, 208KB), which apply to all members and others who use the City’s Leisure Centres.
